Intake form
What an intake form is, what it asks a new client, and how it works online.

Intake form: An intake form is the set of questions a business asks a new client before a first appointment or a first piece of work. At a clinic or a med spa it gathers contact details, health history and the privacy statements the practice asks the client to read; at a law firm it gathers the facts of the matter. It is filled in once and updated when something changes. Online, a client completes it on a phone from a link, and the answers are saved to their record. What the form asks is for the business and its advisers to decide.
Where it shows up
Intake forms are used by med spas, dental offices, clinics and other practices that need a history before they can treat someone, and some booking products send one after a first booking. On a small business website the same words often mean the inquiry form a new customer fills in to start a conversation.
On a site of your own, the intake form often sits behind a client sign-in, so a returning client can see and correct what they told you last time.
Start with a written plan
Tell us about the business and what you need. We reply with questions, then a written plan that lists the work and its price, and nothing starts until you have read it.
